In the foreground, we see the king sitting on a throne, or majesty, wearing a tunic with a crown on his head. He holds a scepter in his right hand and a royal apple in his left. At the foot of the throne is the Kuyavian family coat of arms. The otolith legend is contained in a single line: + WLADISLAVS DEI GRA REX POLONIE CRACOUIE SADOMIR SIRAD LANCIE CVIAVIE. Royal titles are listed, usually placed in the order in which the various lands were part of the kingdom. On the reverse of the medal is an inscription in two lines: PŁOWCE-/-WLOCLAWEK
On the obverse, alluding in imagery to the large majestic seal of Wladyslaw Lokietek, already known from a document of 1320, and later from 1328 and 1333. The artist quite faithfully tried to transfer the image of the seal, which is 10 cm in diameter, while adding, on the sides of the figure, dates indicating the anniversary nature of the medal.

The medal also alludes in its form to seals through an interwoven string symbolizing the silk cords on which the wax sigillum was hung next to documents. Majestic seals were made by the best goldsmiths right at the beginning of each reign. Among the royal seals were the most magnificent and beautiful, as well as the most important and valued. Wladyslaw Lokietek in 1325 explained in one of his documents that he attached a lesser seal to it, because the majestic one was just at the goldsmith's in repair. It is assumed that all kings, beginning with Přemysl II in 1295, possessed them, although so far the seals of several rulers are unknown or not found.
